Analysis on Administration Patterns of Combination Therapies with Quinolones and Multi-Cation Drugs

Abstract
The co-administration of quinolones with multi-cation drugs impairs the absorption of quinolones, and may decrease the efficacy and increase the antibiotic resistance. For this reasons, the KFDA recommends the administration of quinolone 2 to 4 hours before or 2 hours after the intake of multi-cation drugs. Therefore, this study was designed to analyze the administration patterns of combination therapies with quinolones and multi-cation drugs and to evaluate the effective use of quinolones. From January to July 2013, 249 inpatients older than 12 years were included. The study was conducted in Seoul National University Bundang Hospital. The patients were given quinolones and multi-cation drugs on the same day. The co-administration and inappropriate co-adminis- tration of the 2 classes of drugs were retrospectively evaluated. There were 213(16.6%), 172(36%), 695(39.9%) and 216(34.5%) co-administration prescriptions of moxifloxacin, ciprofloxacin 250 mg, ciprofloxacin 500 mg and levofloxacin with multi-cation drugs, respectively. There were 177(13.8%), 143(29.9%), 558(32%) and 139(22.2%) inappropriate coadministration prescriptions, respectively. Dosing intervals for were shorter than 10 minutes for 46% of co-administration prescriptions. Magnesium products were the most common used multication drugs. There were about 31.4% co-administration prescriptions suggesting a decrease in the efficacy of quinolones. Therefore, it is required to educate healthcare professionals about drug interactions and to revise the default standard dosing time for an effective quinolone pharmacotherapy.
